Hospitals & Health Systems
We accept referrals from hospital discharge planners and social workers for patients needing recovery housing.
Case Managers
We collaborate with case managers from various agencies to support their clients' recovery housing needs.
Referral Process
Our streamlined process ensures quick response times and clear communication throughout.
Submit Referral
Complete our referral form with client information and relevant documentation.
Initial Review
We review the referral within 1-2 business days and contact you with initial questions.
Client Screening
We conduct a screening call or meeting with the potential resident to assess fit.
Placement Decision
We communicate placement decisions promptly and coordinate move-in if approved.
Response Time
We understand that timely placement is critical. We commit to responding to all referrals within 1-2 business days, and often sooner. Urgent referrals are prioritized.
Required Documentation
To expedite placement, please have the following ready:
- Valid identification for the client
- Recent treatment discharge summary (if applicable)
- Court documentation (if court-ordered)
- List of current medications with prescriptions
- Emergency contact information
- Insurance or funding source information (if applicable)
